This statement describes this website. It is separate from clinical privacy documents and any formal HIPAA Notice of Privacy Practices.

Contact requests

The contact and callback-request forms send the information you enter to Netlify Forms for processing and storage on behalf of TTP Solutions Healthcare LLC. Authorized team members can review submissions in Netlify. When email notifications are configured, Netlify also sends the submitted details to info@ttpsolutions.org. You do not need to open an email app to submit these forms.

Only basic contact information is requested: your name, phone, email, preferred contact method, and optional non-clinical message or callback preference. Do not include diagnoses, symptoms, medications, records, Social Security numbers, or other sensitive health information.

Email and clinical information

Email is handled by your email provider and the recipient’s email system. This website does not make a HIPAA-compliance claim about Netlify Forms or ordinary email. Contact TTP Solutions Healthcare LLC to confirm the appropriate channel for clinical or sensitive information.

Submitting does not guarantee admission or care. Requests are not emergency communications.

Cookies, hosting, and external links

This implementation does not add analytics, advertising trackers, or form-storage cookies. The hosting provider may process technical information, such as IP addresses and access logs, to serve and protect the website.

Map, resource, and email links open third-party services. Their privacy policies apply when you use them. No third-party map is embedded on this site.

Services are licensed by DBHDS; TTP Solutions Healthcare LLC complies with HIPAA to ensure client privacy. This statement concerns the organization’s services, not a certification of this website’s contact forms.
